Promotion of corporate safety culture
Standing Committee member Susan Wang personally hosted the meeting for promoting the "Blueprint for Creating a Safety Culture" in May 2012, and set out clear tasks and goals for Formosa Plastic Group to create a safety culture. The meeting set 10 awards that are handed out during annual events commending departments with excellent safety culture performance. Chairpersons of the four largest companies personally reward departments with excellent performance, in order to promote the safety culture.

Construction safety management
Contractor construction safety management
To improve the construction safety of contractors, we require contractors' employees to receive safety and health training organized by FPG before entering factories. All workers are required to convene a tool box meeting to take a headcount, verify the mental condition of workers, remind them of construction notices, coordinate work items, and ensure operational safety before commencing construction each day. We appointed safety supervisors who are required to receive 5 days of professional training and pass a strict certification process before being assigned to supervise contractors during construction. This is to ensure the safety of construction workers.
In addition to internal safety and health training, we also co-organized the "Safety and Health Standard Study Tour" with the Occupational Safety and Health Administration, which allows officials from the competent authority, contractors, and employees to engage in exchanges relating to construction safety and concepts, and also obtained good examples and technologies from experts and peers in the industry, thereby elevating our safety and health management standards.

Workplace safety management
FPG adopts international practices and collects excellent management approaches or construction techniques of the industry in Taiwan and overseas, such as hiring a construction safety expert from Mary Kay O'Connor Process Safety Center (MKOPSC) to provide guidance in process safety management (PSM), and a US-based IHS company to provide guidance in process hazard analysis (PHA). FPG also periodically organizes construction safety forums and study tours with labor inspection institutions, and co-organizes emergency response drills with local competent authorities to enhance its safety and health management technologies.

To further improve the results of PSM in recent years and internalize it into a part of our corporate safety value and culture, we signed a safety partnership agreement with the Occupational Safety and Health Administration, Ministry of Labor in August 2017, and created a new image for Mailiao Park in construction safety.
We co-organized a "Study Tour of Safety Partnership Project Implementation Results" with the Occupational Safety and Health Administration, Ministry of Labor in 2022 and 2024, respectively. Besides introducing the goal to reduce major occupational accidents/fire incidents in the park/pipeline leakage by 50% compared with the average five years ago under the framework of the partnership project, we also shared the practical experience of each company in PSM, and gained the recognition of peers in the domestic industry. We organized the "International Process Safety Forum" in Mailiao Park in 2021 and 2023. The Occupational Safety and Health Administration, Ministry of Labor and president of the Group Administration Office jointly hosted the forum, which invited domestic peers, academic institutions, and companies of FPG to send personnel to participate in the forum. Domestic and foreign PSM experts, professors at occupational safety and health departments, and supervisors of insurance companies and the four major companies of FPG to give speeches and engage in exchanges in PSM issues, sharing implementation results and passing down experience in the industry.
Process safety management, PSM
In recent years, we have integrated 14 PSM operations according to standards of the US OSHA. At present, complexes in Taiwan have a total of 227 PSM personnel at all levels, and complexes in China and Vietnam have 79 and 38 PSM personnel, respectively. The personnel are responsible for implementing and controlling PSM operations of each department and ensuring the quality of PSM operations. Furthermore, we began making comprehensive plans for PSM personnel training requirements, course contents, and assessment of training results. As of the end of 2017, we completed the certification of 14 key items of PSM in all parks in Taiwan. We are currently strengthening supplemental certification after changes to PSM personnel, so as to ensure that all PSM personnel have been certified and able to carry out PSM operations.
PSM operations
Process Hazards Analysis, PHA
FPG began commissioning a US-based IHS company to assist in training and certification of PHA facilitators in 2011, and currently has 72 PHA facilitators (57 in Taiwan and 15 in China) certified after training by the US-based IHS company. Regulations for company systems were amended and promulgated in December 2020, and factory offices for each process may be staffed with PHA facilitators responsible for the unit's implementation and quality of PHA work. The PHA facilitators periodically attend risk assessment meetings of the PHA task force of their unit, and provide guidance for operations.
For departments to conduct (semi) quantitative analysis of relatively high risk hazard events after conducting PHA at their workplace, FPH requires further evaluation via layer of protection analysis (LOPA) for level 1 (extremely high risk) and level 2 (high risk) risks, which are unacceptable, found in PHA, to ensure the safety integrity level (SIL) of required safety instrumented systems (SIS). As for factory offices of non-continuous processes, we are promoting the use of failure mode and effects analysis (FMEA) and organizing training, so that it can be used to find potential hazards of important equipment of process units. LOPA and FEMA modules have been created on the risk management platform for the evaluation methods described above, and make our evaluations more consistent and efficient.
Management of Change,MOC
We actively implement process change management to ensure that any changes in design, equipment, raw materials, and operating conditions will not be hazardous to processes, and arranged for the 873 factory (deputy) managers and MOC controllers in Mailiao Park to receive MOC training in 2011. We then arranged for the 16,859 related personnel of departments in all parks in Taiwan to receive MOC training in 2012. We further divided MOC training into basic and advanced courses in 2020, and normalized the courses as part of on-the-job training for related employees.
To improve the quality of MOC, PSM personnel of each department oversee MOC, and certified PHA facilitators of each company provide departments with guidance in MOC in three aspects: "implementation of MOC," "quality assurance for MOC evaluation," and "quality assurance before administrative closure of change," which help departments quickly gain an in-depth understanding of the right way to carry out MOC.
